DTRA Civilian Employees

There are a number of forms you must complete for onboarding. Some forms have to be completed and turned in on the day you start work, and others are due after employment begins. The table below indicates which forms you are required to complete, the order in which the forms should be completed, and any special instructions.  Please review all forms on this page and complete those that apply to you. 

Service Members

You are not required to complete any forms prior to arrival.  All necessary forms will be completed during in-processing with the respective Service team.

Employment Eligibility Verification (I-9) Required for all new employees; you must bring documents on your first day to establish both Identity and Employment Authorization.  This may include any document from List A OR one document from List B AND one from List C.
Employee's Withholding Allowance Certificate (W-4 Form) Required for all new employees
State Tax Withholding Form  Required for all new employees; Select the appropriate form based on your residency.
Self Identification of Disability (SF-256) This form is voluntary unless you have a disability and are being hired under a special authority for hiring individuals with a disability. Your Human Resource (HR) Specialist will let you know if you are required to complete this form.

Ethnicity and Race Identification (SF 181)

Direct Deposit Sign-Up Form (SF 1199A (EG))

Required for all new employees

Statement of Prior Federal Service (SF 144)

Required for employees who have prior military or Federal civilian service.

Designation of Beneficiary, Unpaid Compensation (SF1152)

Required for all new employees

DD214

Required for all veterans; If a DD214 was submitted during the application process, you do not need to bring a second copy on your first day of work.
Life Insurance Election (FEGLI) (SF 2817)                                      

Required for all employees except those on an initial appointment of one year or less, or Term employees who are on intermittent appointments; complete the form, but do NOT sign  until instructed to do so.

 Health Benefits Election Form (SF2809) Required for all employees except those on an initial appointment of one year or less, or Term employees who are on intermittent appointments.

Thrift Savings Plan Election Form (TSP1) 

Required for all employees except those on an initial appointment of one year or less, or Term employees who are on intermittent appointments.

Designation of Beneficiary – Federal
Employees Group Life Insurance (FEGLI) Program (SF2823)
    

Required for all employees except those on an initial appointment of one year or less, or Term employees who are on intermittent appointments.

Thrift Savings Plan Designation of Beneficiary (TSP3)  

Required for all employees except those on an initial appointment of one year or less, or Term employees who are on intermittent appointments
Designation of Beneficiary  –  Federal Employees Retirement  
System (FERS) (SF3102)
Optional for employees covered under the Federal Employees Retirement System (FERS).
